What is Sciatica?
Sciatica describes symptoms caused by irritation or compression of the sciatic nerve which is the largest nerve in the body, running from the lower back through the buttock and down the back of the leg. When compressed or irritated, it produces pain, numbness, tingling or weakness anywhere along that pathway.
Lumbar disc herniation: The most common cause. A disc bulges and presses on the nerve root. Usually responds well to physiotherapy.
Piriformis syndrome: The piriformis muscle compresses the sciatic nerve in the buttock. Responds extremely well to dry needling and targeted stretching.
Spinal stenosis: Narrowing of the spinal canal compressing nerve roots. More common in older adults. Physiotherapy significantly improves function.
Facet joint irritation: Can refer pain into the buttock and upper leg, which is often mistaken for true sciatica.
How We Treat Sciatica
Treatment begins with identifying the actual source of nerve compression. This changes the approach significantly — disc-related sciatica is managed very differently to piriformis syndrome.
Neural mobilisation: Specific movements that gently mobilise the sciatic nerve through its full pathway, reducing irritation.
Lumbar manual therapy: Joint mobilisation to reduce compression on the nerve root and restore movement.
Dry needling: Targeting piriformis and gluteal trigger points contributing to nerve compression.
McKenzie method: Directional exercises specific to the type and direction of disc herniation. Often dramatically reduces leg symptoms.
Progressive exercise: Building core and lumbar muscle strength to support the spine and reduce load on the affected structure.

Short rest periods during the acute phase are fine but prolonged rest is counterproductive. Movement within your pain limits keeps the nerve mobile and prevents deconditioning.

No. The vast majority of sciatica resolves with conservative physiotherapy management. Surgery is considered only when there is severe neurological deficit or conservative treatment has genuinely failed.

True sciatica involves nerve irritation producing burning, electric or shooting pain with numbness or weakness. Referred back pain is muscular and produces a duller, aching sensation. The distinction matters for treatment.

Sustained sitting is a major contributor to disc herniation which is the most common cause of sciatica. If your job involves long sitting periods and you have developed leg pain, this is very likely a contributing factor.
